Bradford Health Services provides the highest quality in community-based aftercare for loved ones and friends. We provide transition services to individuals or families who need assistance moving from a short-term residential service to a longer term residential service, such as a skilled nursing facility or a full time nursing home. These services are geared to meet the needs of people who may need housing after a period of living on the streets or other out of the way locations. Our goal is to help the client get the resources they need to stabilize their lives while they prepare for a more permanent life in a long term care or residential facility. Our services also include short-term supportive services for those individuals or families who have already made the decision to move forward and are looking for the best place to do that.
Our transition services work with the client to establish a clear plan for transition. This plan should include a clear plan of when the individual or family will enter the program, what they need to accomplish there and how they can maintain their personal hygiene while maintaining a connection with their support network. Our goal is to prepare individuals for transition by providing training and tools, so they can maintain consistent access to their resources during the transition period. There are several programs for both individuals and families.
The transitional living program is provided in an atmosphere that is equipped to support the client’s needs. It is a two level program; the transitional living center has the dual goal of transition services for individuals or families, and a transitional care unit that provides in-home services to families or individuals. Some of the transitional care units are in residential homes and some are in community-based centers. Either type can be very effective for the client. Some of the services provided at the transitional living center may include drug or alcohol addiction assistance, medical treatment for addictions or other conditions, employment and training services, and referral to local agencies that can help with other needs, such as elder law firms, legal aid organizations, and other agencies that can assist the client after leaving the program. The goal of the transitional living center is to provide a secure environment in which the client can achieve the goals they’ve set for themselves.
