Destiny Sober Living is a full-service, transition services organization that offers a complete range of on-site and off-site services to meet the needs of individuals, couples, families and individuals in crisis. The organization provides the following services: long-term care transition planning, direct care transition services, elder-care assistance, transition home care, individual and family therapy, respite care, life support systems, and respite services. They do not provide medical services but instead, offer a network of doctors and other healthcare providers, including psychiatric and drug abuse specialists, to coordinate the transition of loved ones. The goal of Destiny Sober Living is to help families and individuals to achieve permanent living in a safe and secure environment.
When you join Destiny Sober Lives you are assigned a Transition Team made up of licensed, certified counselors who will meet with each family to discuss the needs and preparation for transition. A Transition Team will include a trained counselor and family member who will work with each family individually to determine the level of transition preparation required. During your initial meeting with the Transition Team you will be provided with a variety of resources, including support and referral sources. They will also provide you with a list of resources throughout the duration of your stay.
During your first week at the transitional living facility you will be given a kit that includes: a prescription pad, prescription book, emergency contact information, medications, and identification. You will also have a copy of your family’s medical history. The first nights and weekends will offer you the opportunity to get to know the staff and fellow residents. As you begin the transition into your new living situation, you will meet and become comfortable with the other residents while working toward creating a solid support system for you and your family.
